Medical Privacy & Confidentiality Statement
At The Wellness Group LLC (hereinafter The Group), we recognize that the protection and confidentiality of medical and health-related information and other personal data is essential, and we are committed to this purpose in accordance with all applicable legal requirements.
This statement discusses how The Group gathers, uses and discloses your personal health and other information, as well as The Group’s policies and practices for keeping information private and secure. The Group urges you to review this policy carefully and to check back periodically in case there are revisions.
If you use The Group’s website and/or services therein, you acknowledge and agree to the terms of this statement, and to the fact that they may be modified from time to time as provided above.
Your Personally Identifiable Information
To serve your health concerns, wants and needs, The Group collects information directly from you on the website, or orally through the telephone using forms. If you provide The Group with express, written permission and/or legal authorization, The Group may also gather information from third parties such as your health care providers or physicians.
All health information which identifies you is known as “personally identifiable information,” and it is shared with The Group’s network of hospitals, clinics, recovery, healing and therapeutic centers; doctors, dentists, and any other health care professionals who may treat or care for you in order to provide you with the correct health care services and solutions. Your personally identifiable information will be used only as necessary to provide the services that you request or as otherwise required by law.
The Group is not comprised of medical professionals. As such, The Group will not evaluate the information that you provide, and The Group will not advise you based on the information that you submit or that you arrange to have submitted to us.

Security Concerns and Precautions
The Group has implemented reasonable precautions to ensure that your personally identifiable information is retained in a secure fashion, protecting it against loss, misuse and unauthorized access, disclosure, alteration or destruction. The Group utilizes leading-edge Secure Socket Layer (SSL) encryption, firewalls, anti-spyware, and other intrusion detection systems so that your data will remain safe and secure.
The Group only permits authorized employees to access your personally identifiable information and said employees may only use said infornation for business purposes authorized by The Group. As effective as The Group’s safeguards and precautions are, it is no secret that no security system is completely impenetrable or 100% failsafe and tamper proof, so we cannot guarantee the absolute security of our database against willful intrusion, nor can we guarantee that the personally identifiable information you supply will not be intercepted by third parties while it is being transmitted to the Group over the Internet.
However, The Group is committed to your privacy and security, and we will not sell or otherwise transfer your personally identifiable information to another party for any purpose whatsoever, except to those who provide the services that you request or authorize The Group to request on your behalf.
In the unlikely event that assets relating to The Group’s website are sold or transferred, your personally identifiable information may be transferred to this acquiring entity, and it will be the acquiring entities responsibility to safeguard your personally identifiable information.
Finally, The Group may disclose your personally identifiable information to third parties if it is believed that existing law requires disclosure in order to be in compliance (this includes responding to court orders and subpoenas).
Non Personally-Identifiable Information
If you do nothing during your visit but browse through The Group’s website, read pages, look at the videos, or download information, The Group will automatically gather and store certain information about your visit that does not identify you personally (“non personally-identifiable information”). Non personally-identifiable information may include but it may not be limited to: Your IP address, the domain name and browser that you used to access the Internet, the pages that you visited on The Group’s website, and the address of the website that you visited immediately prior to visiting The Group’s website.

THE GROUP DOES NOT TRACK OR RECORD INFORMATION ABOUT THE IDENTITY OF INDIVIDUALS AND THEIR VISITS TO THE WEBSITE.
Sometimes a small file on the user’s computer called a “session cookie” will be written during a visit. Session cookies automatically expire when visitors leave the website; session cookies retain information only during the session, or for the purpose of completing a particular online transaction, without any ability or capacity to track users over time and across different websites.
The Group is also formally committed to protecting minors, and no information is collected, stored, or disclosed about children under the age of 18, without prior parental consent. If The Group receives information from anyone under the age of 18, such information will only be used to inform the child that The Group requires parental consent.
